Population Health Management (PHM) is an innovative model of care delivery that aims to improve the health of patients while lowering the overall costs of care. PHM includes best practices and tactics to enhance the patient experience, improve outcomes, and provide better organized and more personalized care to our patients.
PHM is seeking a Project Specialist to assist with the Medicaid ACO team. Looking for a detail oriented, persistent and organized team player. The Project Specialist will provide administrative management and support for the MGB Medicaid ACO contract. This includes, as applicable, learning about and assessing the ACO programs and models, supporting participation and other strategic decisions within the MGB organization, and working with clinical and administrative leadership to help maximize MGB's performance. The position also provides support for ACO operations for primary care practices.
Principal Duties and Responsibilities
• Provide organizational support for overall Medicaid ACO contract requirements. • Translate certain contract requirements into summary materials and timelines for staff education • Prepare presentations, meeting materials, minutes, and action plans. • Work closely with MGB Community Health, MGB Data and Analytics Organization (DAO), and MGB Population Health Management program staff. • Manage the day-to-day operations of the PHM Fellows and Residency Programs. • Lead the Medicaid ACO operations around care needs screening. • Manage MassHealth reporting requirements as needed. • Maintain library of Medicaid contract and amendments, Medicaid bulletins, and MGB's required submissions to the state; manage Lumos Central Resource Hub. • Assist practices in managing their MassHealth ACO membership and panel sizes. • Work with analysts in DAO to understand structure of our data; in the future, perform high-level analysis using Tableau, our Medicaid dashboard. • Represent Medicaid ACO Team on projects or at meetings as needed. • Support ADT (Admissions, Discharges and Transfers) platform. Work with Regional Services Organizations (RSOs) and our ADT vendor to provide access and training for the ADT portal, answer questions and troubleshoot issues. • Manage process to maintain all practice information with MassHealth; manage referral circle for the ACO, and regular reconciliation of all practice data. • Support other departments with the operations of MassHealth programs, as assigned. • Provide assistance on additional projects as assigned.
Qualifications Qualifications • BS/BA or equivalent required • 1-3+ years' experience required, preferably in a healthcare environment • Experience providing project management support required, preferably in a healthcare setting • Experience with PC / Microsoft applications including Word, Advanced Excel, and PowerPoint • Master's degree preferred, but not required
• Strong organizational, administrative, and project management skills • Ability to prioritize and resolve critical issues efficiently and effectively • Ability to effectively present ideas and information • Strong analytic skills; ability to turn data into information • Strong written and verbal communication skills • Strong commitment to customer service • Experience with Microsoft Office applications; advanced Excel skills • Ability to work independently
