General Summary/Overview Statement: Partners Specialty Pharmacy is a not-for-profit wholly owned subsidiary of Partners HealthCare. The Partners Specialty Pharmacy will serve Partners Healthcare patients from a centrally managed location in the Boston area with support from local entity clinics and pharmacy departments. The Partners Specialty Pharmacy will deliver high quality services with an emphasis on patient education, service excellence and comprehensive care management. Partners Specialty Pharmacy will serve as an integral link between patient and provider, efficiently providing first-rate service to optimize patient outcomes and achieve organizational financial goals. Therapeutic areas of focus will include Hematology, Oncology, inflammatory conditions, Hepatitis C, HIV/AIDS and Multiple Sclerosis. GENERAL SUMMARY / OVERVIEW: Provide medication management through an evaluation of medications for patients to ensure safe, effective utilization that is tailored to specific medical conditions. Work with patients, doctors, nurses and other health care professionals to avoid problems associated with drug therapy and ensure optimal patient outcomes. PRINCIPAL DUTIES AND RESPONSIBILITIES: Practice Involvement: • Maintain proficiency in the skills and theory of the practice of pharmacy by reviewing literature, attending in-services, seminars and conferences • Actively participate in our clinical meetings Provide Pharmacist services to patients: • Ensure all drugs are appropriately indicated, effective, safe and convenient for the patient • Identify, resolve and prevent drug therapy problems • Ensure that the patient understands the specific goals of therapy and that the patient and providers are working to achieve these goals • Evaluate and document patient outcomes at each encounter • Provide patient education on medications and disease states in individual or group settings Perform initial patient evaluation as it pertains to the prescribed therapies: • Collect analyze, interpret and evaluate appropriate data relative to medication therapy • Obtain histories and apply these along with other patient data in the pharmaceutical care evaluation • Identify, resolve, and document patient specific drug therapy problems and issues in the patient's best interest • Evaluate the significance of signs and symptoms relative to disease processes and make recommendations for the modification of drug therapy • Evaluate the significance and clinical importance of adverse reactions and complications associated with drug therapy • Provide counseling and answer questions from patients, family, and health care professionals concerning drug therapy • Provide dispensing Work on-call wearing pager on a rotating basis with other pharmacists Demonstrates Commitment to Customers: • Is aware of different customer needs/desires • Takes action to meet customer needs/desires with respect to customer diversity • Helps improve processes to meet customer needs. • Suggests and acts on ideas to improve overall customer service • Respects confidentiality and shares information with only those that need to know Demonstrates Commitment to Fellow Employees: • Seeks to understand and value differences between employees • Treats other employees with dignity and respect • Demonstrates open, honest communication and behavior • Resolves conflict in a positive manner • Contributes to the productivity of the workgroup Demonstrates Commitment to Health System: • Lives the values of dignity, integrity, service and compassion • Supports the mission/vision • Speaks positively about Health System • Chooses to make Health System a better place • Use/s the Partners HealthCare values to govern decisions, actions and behaviors. These values guide how we get our work done: Patients, Affordability, Accountability & Service Commitment, Decisiveness, Innovation & Thoughtful Risk; and how we treat each other: Diversity & Inclusion, Integrity & Respect, Learning, Continuous Improvement & Personal Growth, Teamwork & Collaboration • Other duties as assigned Qualifications QUALIFICATIONS : • Degree from an accredited college of pharmacy • Level of education preferred for this job: Pharm D • Experience (2 years) managing patients' specialty medication needs or, • Clinical Pharmacist hospital practice experience (2 years) preferably in one or more specialties • Excellent verbal and written skills • Proficiency with Microsoft Office SKILLS/ABILITIES/COMPETENCIES: • License/Certification/Registration: Current State pharmacy license • Preferred License/Certification/Registration: CSP • Minimum Experience: Three years' experience as a clinical pharmacist with direct patient care • Preferred Experience: Specialty Pharmacy • Additional Requirements: Excellent customer service and communication skills, proficient in the English language, ability to manage time and people effectively, good attention to detail, ability to follow policies and procedures.
