This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. OR Education: Graduation from an approved school of at least 24 months duration. This position requires the raising of objects from a lower position to a higher position or the movement of objects horizontally position to position. Lifting may be done to move a patient from one location to the next, i.e.- from the exam table to the chair, and in the movement of equipment or supplies. This position requires exerting up to 50 pounds of force occasionally and/or up to 25 pounds of force frequently to move objects. This position requires writing or otherwise working with fingers and/or hands, including one or both hands frequently. This is done when taking a patient's pulse, utilizing a stethoscope, administering medications, entering information into the computer, etc. This position requires substantial movements (motion) of the wrists, hands, and/or fingers, as is done when entering patient notes into the computer. The Licensed Practical Nurse (LPN) provides clinical care including lab services at a Community Based Outpatient Clinic (CBOC) under the Aleda E. Lutz VA Medical Center, Saginaw, Michigan. The LPN provides a wide range of clinical services to the veteran that demonstrates complex technical judgment under direct supervision of a registered nurse or Licensed Independent Practitioner (LIP). The LPN utilizes basic nursing skills and techniques, of varying degrees of complexity, in a general care setting, to provide personal and general nursing care to adult patients, aged 18-85 and older, who require treatment for common medical conditions. The PACT Float LPN may be assigned primary care and lab duties at multiple CBOC locations including completing direct patient care and lab duties such as specimen collections and processing. In addition, the PACT Float LPN may be assigned to primary care duties using utilize Clinical Video Telehealth equipment to facilitate primary care remote visits. The LPN participates in data collection, implementation, and evaluation of care for all ages of patients treated. In care of the elderly, planning and implementation is aimed at providing care to support and reduce the effects of the aging process, health maintenance and promotion, minimizing adverse effects of drugs, and maintenance of optimal psychosocial functioning, including assisting with the management of confusion and disorientation. The Primary Care LPN will not be assigned to a specific Patient Aligned Care Team (PACT) and will cover primary care and lab duties at the CBOCs requiring the incumbent to drive in a government vehicle. Duties include but are not limited to: Performs all the functions of the LPN, and in addition, carries out more specialized patient care duties, while functioning within the scope of practice. The LPN completes nursing tasks independently with minimal need for direct oversight via RN or LIP. Collects and processes lab specimens per LIP and/or nursing standing orders; Correctly accessions, labels and processes specimens for shipping; Performs phlebotomy, lab specimen collection, processing and packing following DOT requirements for safe quality specimen transfer Administers medication accurately and promptly under the general supervision of a registered nurse or LIP. Demonstrates understanding of the effects and/or reactions caused by medications, treatments, and procedures. Records and reports these observations. Performs basic physiological measures as ordered and PRN (e.g., temperature, pulse, respirations, blood pressure, pain, diabetic foot screen, height/weight, pulse oximetry, doppler). Demonstrates ability to make astute observations and thoroughly documents care and patient response. Assists the RN in evaluating changes in patient condition.
