This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Persons hired or reassigned to Senior Social Worker positions in the GS-0185 series in VHA must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: Senior Social Worker, GS-0185-12 Experience and Education: Two year of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. In addition to the experience above, the candidate must demonstrate all of the following KSAs:(a) Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management.(b) Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice.(c) Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes.(d) Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills.(e) Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area. The incumbent in this HCHV outreach position provides outreach services to Veterans struggling with issues of homelessness in a variety of community settings. Assignments involve providing continuing social work services at a field location alone or with other HCHV staff members. Such assignments typically require extensive coordination of services with a wide range of VA and community partners. The HCHV Outreach Worker independently performs outreach in the community and facilitates eligibility determination by securing military discharge papers when necessary and facilitating enrollment for care with VA eligibility staff and determining HCHV eligibility in accordance with the HUD definition of homelessness. The outreach worker independently conducts psychosocial assessments; develops treatment plans in collaboration with the Veteran/family and with the interdisciplinary treatment team. The outreach worker possesses the knowledge and skills to independently implement treatment modalities and provide counseling or psychotherapy for individuals, families and groups. The outreach worker possesses the skills to perform psychosocial interventions and measure outcome results. The outreach worker is proficient in coordinating community based services, including information and referral for additional services from other VA programs, other government programs and community agency programs. The outreach worker independently identifies high risk patients and provides case management services. The outreach worker provides crisis intervention services, seeking to address the cause as well as presenting complaint, engaging family and other support systems as appropriate. The outreach worker has functional knowledge and experience in the use of medical and mental health diagnoses, disabilities and treatment procedures, including acute, chronic and traumatic illnesses and /injuries, common medications and their effects, side effects, and medical terminology. The official duty station of the HCHV Outreach worker is located in the community at the CRRC for Homeless Veterans in Huntington WV. The work of the HCHV Outreach worker is conducted in the community. Veterans are typically contacted at missions, soup kitchens, shelters and other street locations as well as halfway houses, rehab centers, and other treatment facilities. Incumbent must provide outreach to homeless Veterans in \"dangerous areas\" (many times unsafe and/or unsavory), which may be without the benefit of police protection. Irregular hours may be required, as Veterans' needs and availability does not always correspond to an 8:00am to 4:30pm work schedule. A station car will be made available for community work. It is a requirement of this position to drive a government vehicle and be cleared to transport Veterans in need.
