The Homeless Patient-Aligned Care Team (PACT) Coordinator, coordinates the program and works with the other members of the multidisciplinary PACT, performing outreach to identify Veterans who are homeless or marginally housed, with the goal of engaging them in primary care. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
