The Health Department is seeking a Public Health Nurse IV to provide programmatic oversight of the Health Integration Team within the Division of Health Services. Under the direction of the Assistant Division Director, the Health Integration Team Manager provides programmatic oversight of the Health Integration Team within the Division of Health Services. Responsible for budget development, setting program priorities, assessing workload demands, and coordinating assignment of staff within multiple primary care settings. Works with department and community partners to strengthen the County's safety net programs, assuring coordinated relationships between Health Department staff, local Federally Qualified Health Centers (FQHCs), and other community partners. Necessitates continuous work with all Health and Human Service (HHS) Departments to promote continued integration of primary care, behavioral health, oral health and specialty care. This position serves as the department lead on a multi-agency team, in the development of an HHS-wide care coordination model.

MINIMUM QUALIFICATIONS: Graduation from a college or university accredited by the National League for Nursing or the Commission on Collegiate Nursing Education with a bachelor's degree in nursing; plus five years of post-licensure experience as a nurse in an equivalent health care setting whose focus was primarily prevention services and community education, including three years in a public health/community health setting. Qualifying experience may include experience in ambulatory care settings such as outpatient clinics that incorporate community outreach/education as part of services or specialty settings that incorporate teaching, community outreach, resource referrals, etc. as part of a health care continuum; plus at least one year of supervisory experience.
CERTIFICATES AND LICENSES REQUIRED:
Possession of current license or a multi-state licensure privilege to practice as a Registered Nurse in the Commonwealth of Virginia
NECESSARY SPECIAL REQUIREMENTS: The appointee to this position will be required to complete a criminal background investigation, a child protective services, sanction screenings, and a driving record check to the satisfaction of the employer.
PREFERRED QUALIFICATIONS: Master's degree in nursing with three or more years of experience in a public health/community health setting. Two or more years of experience working with culturally and ethnically diverse clientele. Must have the ability to take initiative in developing intervention strategies designed to promote the health and social needs of the population served. Ability to demonstrate strong leadership, supervisory, and interpersonal skills. Must be able to lead a multidisciplinary team and work collaboratively with clinical staff to best address the medical and social needs of clients served.
PHYSICAL REQUIREMENTS: Employee is required to walk, stand, sit, bend, stoop, kneel, reach, climb stairs and carry equipment or young children up to 25 pounds in weight. Uses hands to grasp or feel. Visual acuity is required to read data on a computer monitor. Ability to operate keyboard driven equipment. Ability to drive a vehicle. Position frequently communicates and must be able to exchange accurate information with others verbally and in writing. All duties performed with or without reasonable accommodations.
